Any program is only as good as its focus on
the individuals the program serves.
Case Management
In assisting participants in preparing to navigate through transitional seasons of their lives, Case Management Services are paramount.
Participants that engage in most of our programs or services, specifically those returning to the community from residential placement, jail, or prison, are assigned a Case Manager.
Regular assessments and engagement will be continued as participants begin to apply the information, skills, and tools learned throughout this walk and move toward a change in their thinking process. Considerable engagement will begin to be laser focused on the various immediate needs of participants:
- substance abuse
- educational pursuits
- vocational training interest and/or possibilities
- workforce development (soft skills)
- financial literacy
- health & wellness
We will collectively identify current strengths and weaknesses of participants as well as community resources and partner organizations to make them aware of any additional programs, services, and community opportunities, and assist them in getting connected as they move toward independence.
